Dr. Chilukuri frames melasma as his "most dreaded condition" — a chronic problem he likens to diabetes: it may clear, but it comes back, so the goal is maintenance rather than cure. He explains the role of senescent fibroblasts (the "zombie cells" trending on TikTok) that concentrate in hyperpigmented patches. These aren't dead or apoptotic, just "lazy" and aged. Non-thermal microneedling or pulsed-wave radiofrequency microneedling can reactivate them without generating the heat that would switch on the pigmentation pathway, while healing the DEJ at roughly 300 microns (0.3 mm) increases anchoring fibrils, decreases transepidermal water loss, and reduces environmental inflammation.

He lays out three treatment goals — control external inflammatory triggers (UV, pollution, blue light from screens), correct pigment quickly (chemical peels, picosecond devices, pulsed radiofrequency), and prevent recurrence by repairing the skin barrier. On sunscreens, physical blockers like zinc oxide and titanium dioxide are "non-negotiables," tinted formulas reflect more, and newer products with Physalis angulata (wild gooseberry) or pycnogenol stop heat shock protein from turning on melanocyte-stimulating hormone. He also highlights cysteamine as a hydroquinone alternative needing no drug holiday, and exosomes for down-regulating inflammation.
