Serota, who is triple board certified in allergy/immunology, pediatrics, and dermatology, explains how he gives patients "30 seconds on the pathophysiology" using a military analogy: the immune system is like an army, and one battalion has gone rogue. If you "jam the radio signals" of just that battalion, you can shut off the disease process while leaving the rest of the military to do its job — the soldiers are all still there waiting for a go signal that never comes. He argues this framing helps patients understand how a treatment works and improves compliance.
He also addresses what to do when inflammatory skin diseases have overlapping features. Rather than staying stuck, he advocates hedging your bet on the pathway you think is right — eczema/dermatitis versus psoriasis — targeting that pathway, then reassessing at week 12 or 16 to see if the patient improved. In murky cases, "the treatment can prove your diagnosis." On urticaria, he prefers the label chronic autoimmune urticaria, noting that 30–50% of chronic cases have autoantibodies triggering mast cells independently of any external trigger, consistent with the two-to-one female predominance seen in autoimmune conditions.
